  1. Please complete the following form completely and accurately (please be sure to include street, city, state, and zip with all addresses).  This information will be used for both the online directory of active PNSA clubs, and published in the PNSA Competition Guide.

  2. You will also need to mail your annual club dues of $85 (the club dues includes one club representative's registration to attend the annual PNSA convention in May):
